Ensuring that Health IT-generated reports and data reflect an accurate picture of the care and outcomes of your population ensures that data is actionable for quality improvement, monitoring as well as many other purposes. This health center learning collaborative series presented health center case examples that explore the privacy and data sharing considerations of EHR documentation of sensitive patient information, such as social history and social risk, and encouraged participants to discuss the implications for health centers and their patients.

Session 5, April 6, 2022: Considerations when Documenting Risk of Acquiring HIV and/ Gender Identity and Sexual Orientation

The HITEQ team provided a review of key considerations on coding and documenting social information that were raised during presentation and discussion of case examples and facilitated discussion on application of the considerations within participating health center’s unique contexts. Participating health centers shared their takeaways and how those takeaways may be enacted in the clinic.
