Established in 1995, CHAN has developed and supports an integrated clinical and administrative system infrastructure for its ten Federally Qualified Health Center (FQHC) members which includes 3 Healthcare for the Homeless Programs. Our mission: To enable our member agencies to develop the program and resources necessary to assure access to efficient, effective quality health care for all clients in our communities, particularly the uninsured, Medicaid, and medically underserved populations.
