Are you interested in learning more about how your Health Care for the Homeless (HCH) health center can address positive social need screens?

Join the National Health Care for the Homeless Council and the HITEQ Center for a free webinar on Tuesday, October 22nd, 2024, from 3-4 pm Eastern Time (2-3pm Central) and learn how to support timely social needs referrals and care planning and operationalize social needs data. Presenters will provide guidance on optimizing social needs screening workflows, including those housed in Electronic Health Records (EHRs), for following up on positive screens and effectively using the collected data. A panel of expert health center representatives will share examples of how these workflows can be applied. While this webinar will focus on HCH health centers, anyone involved directly in social needs screening processes are welcome.


Learning Objectives
After this webinar, attendees will:
1. Evaluate their organization's social needs screening workflow based on promising practices and peer health center examples.
2. Describe strategies that could be applied in their health center to improve follow-up to social needs screening and monitoring of social needs data.
3. Identify which strategies can be introduced at their health center.
