In the first session, participants will explore the role of CHWs in data-driven activities essential to value based care, including outreach and proactive care management, and how these rely on successful use of the health center's EHR and other systems. The second session will feature a conversation with CHWs, sharing their challenges and successes in becoming team members in the current landscape of value based care.
