Resource Overview

Patient portals, sometimes also referred to as personal health record systems (PHR) are web-based portals commonly attached to electronic health record systems (EHRs). These patient-centered portals provide patients with the ability to login and review health information related to their care. Common patient portal services include ways in which to schedule appointments, send messages to their care providers, review test results and refill prescriptions.

Outside of the benefits to the patient, implementation of patient portals had come to the attention of healthcare providers due to the inclusion of Meaningful Use of objectives centered on the use of patient portals and electronic engagement with patients.  Stage 3 requirements are still being explored and the impact it will have on Health Centers is unknown. Therefore, it is a challenge for small practices and Health Centers to determine how to best derive value from Patient Portals and effectively implement them into their workflow.

The tools and articles posted below are meant to provide examples, templates and strategies that can assist Health Centers in understanding how patient portals can better engage their patients in self-management of their care, and after an initial investment in time and money can decrease the burden on their clinical and administrative staff.

The Arizona Alliance for Community Health Centers (AACHC) is the Primary Care Association (PCA) for the State of Arizona since 1985. All states have one designated PCA to advance the expansion of the Health Center Program and advocate for the health care interests of the medically underserved and uninsured. AACHC, as a result, has many programs to assist member community health centers and the disadvantaged populations they serve. These programs vary from centralizing financial information and educational opportunities for members to peer networking committees for sharing of best practices.

MOQuIN was formed in 2010 with HRSA Health Center Controlled Network funding. All Missouri health centers are members of MOQuIN and staff representatives meet on a routine basis to discuss best practices and opportunities for quality improvement. MOQuIN provides support for MPCA’s member health centers to leverage Health Information Technology (HIT) to improve patient care and report on clinical quality measures, and achieve Meaningful Use of HIT to receive the incentives available through the American Recovery and Reinvestment Act (ARRA). Use of the data warehouse, population health management software, and staff expertise enhance the success of this network in organizing statewide quality improvement efforts and initiatives.

Incorporated in 1987, the Montana Primary Care Association is a non-profit organization dedicated to improving access to high-quality, community based, affordable primary health care in Montana.  MPCA is the association of Montana’s community health centers, and works to support and increase Montanans’ access to excellent, patient-centered health care. MPCA provides training and technical assistance to health centers and advocates for policies and programs that improve access to health care for all, especially for underserved and vulnerable populations.

Founded in 1980, the New Mexico Primary Care Association (NMPCA) is a non-profit 501 (c) 3 corporation, representing 19 member organizations that operate over 160 primary care, dental, school-based and behavioral health clinics, throughout New Mexico. NMPCA also operates a HRSA-funded Health Center Controlled Network composed of 12 New Mexico and 3 Utah Health Centers. Doing business as New Mexico Health Choice Network, NMHCN assists members to generate and use data and analytics to achieve meaningful use, pursue Healthy People 2000 clinical quality goals, and support patient centered medical home implementation and certification efforts.  Additionally, NMHCN has trainers on staff, certified by eClinicalWorks in both Practice Management and Electronic Medical Records. They are available to provide training and super-user services to 11 health center members utilizing that software.

Formed in 1978 by the leadership of community health centers, NCCHCA is comprised of membership from each of the 40 health center grantees, 1 Look-alike organization, aspiring health centers and other partners. NCCHCA was created to promote the common mission of health centers across the state. They represent the interests of North Carolina’s health centers to federal, state, and local agencies and officials. NCCHCA is a valuable resource to health centers, providing training and technical assistance in areas such as clinical service delivery, governance, workforce development, and administration. NCCHCA regularly presents workshops, trainings, and conferences to keep health center staff on the cutting edge of effective and cost-efficient service delivery. The Association consistently analyzes key issues facing health centers and provides our members with critical information in a timely fashion. In addition, the Association helps communities to create new health centers or expand existing ones.

Founded in 1994, The North Coast Clinics Network (NCCN) is a consortium of community health centers working to improve the Health Outcomes of our under-served populations. NCCN’s member clinics and health centers are working together to improve access to high quality medical, mental and dental health care for all people, regardless of their ability to pay, through information sharing, community education , shared administrative activities and direct service projects. The North Coast Clinics Network’s thirteen Federally Qualified Health Centers (FQHCs) member organizations span an area that encompasses three rural counties in the Northwest corner of California.

Founded in 1999, OSIS is one of the few full-service Health Center Controlled Networks in the country. OSIS is custom-built to solve the unique technology challenges faced by Community Health Centers. And unlike other vendors, OSIS is a 501(c)(3) non-profit organization—just like the members they serve. And because they are a Health Center Controlled Network, over half the Board is comprised of OSIS members—giving the Health Centers served the opportunity for true partnership and access to training, resources and lessons learned from other centers in their network. This shared knowledge is invaluable for saving their members time, frustration and the uncertainty of navigating today’s complex technology challenges on their own. They are also one of NextGen Healthcare’s largest customers. We understand its products’ “out of the box” limitations for Health Centers—and how to overcome them to make our members’ technology investments (and staff) as highly functional as possible.
