During this webinar, the Centers for Medicare & Medicaid Services (CMS) will discuss the new Preview Reports for Hospices that will be available to providers in the near future. Participants will gain an understanding of how to access these reports, how to interpret the contents of these reports, and what to do if they believe their report contains an error.

This Enabling Services Implementation Packet, from AAPCHO, serves as a guide for health centers who wish to codify and track enabling services. The packet was developed as a standardized data collection model to improve data collection on these essential services, and better understand them and their impact on health care access and outcomes.

This tool is for use by health centers interested in assessing their readiness for Patient Centered Medical Home (PCMH) recognition by the National Committee for Quality Assurance (NCQA) under the 2014 Standards. It can be used by first time users as well as those seeking a renewal. The tool walks organizations through the requirements for each of the standards, elements, and factors for NCQA PCMH recognition by providing a way of tracking what they currently have in place and what they are missing, a list of supporting materials to submit to NCQA, as well as Health IT tools that can support both the implementation of the required factors as well as the generation of the documents/reports needed for submission.

In this article the authors describe the rationale for extracting data on social determinants of health from EHRs, including the potential influence of aggregated data on quality improvement activities and health care payment reform.
