With 27 health centers in nine states serving approximately one million patients, Health Choice Network, Inc. is recognized as a leader in the integration of health information technology among health centers and safety net providers.

Founded in 1983 as a membership organization of Federally Qualified Community Health Centers, the Health Federation of Philadelphia continue to partner with members to improve the quality and accessibility of primary health care, including behavioral support, for vulnerable populations in Southeastern Pennsylvania. They support the functioning of members with collective advocacy, professional development, individualized consultation, and shared learning.

Healthy Communities Collaborative Network (Arizona Alliance for Community Health Centers)

The Arizona Alliance for Community Health Centers (AACHC) is the Primary Care Association (PCA) for the State of Arizona since 1985. All states have one designated PCA to advance the expansion of the Health Center Program and advocate for the health care interests of the medically underserved and uninsured. AACHC, as a result, has many programs to assist member community health centers and the disadvantaged populations they serve. These programs vary from centralizing financial information and educational opportunities for members to peer networking committees for sharing of best practices.

Established in 2011, The Health Center Association of Nebraska (HCAN) is a nonprofit primary care organization whose mission is to enhance and expand access to quality, community-responsive healthcare for Nebraska’s medically underserved and uninsured. Members of HCAN are federally-qualified Community Health Centers (CHCs) that provide medical, dental, and mental health services for over 63,000 Nebraskans.

Since 1983, the Idaho Primary Care Association (IPCA) has been the leading state advocate for community-based health care programs. The association plays a vital role in educating federal, state and local policy makers about issues relating to health care and the role of community health centers. The IPCA also provides training and technical assistance to Idaho's community health centers in the areas of community development, quality improvement, workforce development and health center operations.
