CHIP, Inc. was founded in 1997 as the only statewide network of community health centers in the State of Maryland. As a leader and advocate for quality community health care in Maryland, CHIP provides community health centers with the business expertise to achieve the shared goal of quality improvement in the care they deliver.  In doing so, we give these centers access to higher levels of sophisticated health information technology (HIT) and management services than they could achieve and maintain individually.

Founded in 2013 The Federally Qualified Health Center Urban Health Network (FUHN) is an organization of ten Twin Cities FQHCs working in partnership with Minnesota’s Department of Human Services on Medicaid health care reform efforts.  These efforts include FUHN’s participation in a Integrated Health Partnership Accountable Care Organization demonstration project. Through this demonstration project FUHN participant FQHCs hope to further enhance the health care provided to their Medical Assistance patients by coupling our primary care efforts with expanded use of technology.

Georgia Association for Primary Health Care

The Georgia Association for Primary Health Care, Inc. (GAPHC) is a membership organization for all Federally Qualified Health Centers (FQHC) in the state of Georgia. GAPHC was founded in 1977 and is a statewide organization located in the Atlanta Metropolitan Area. GAPHC is quite familiar and knowledgeable about the relevant rules and regulations governing FQHC – Section 330 organizations. GAPHC is very active in the development of various operational models for FQHC organizations and provides direction and leadership for required community development activities that are related to both the development of new start FQHC organizations and operations of existing FQHC organizations.

Hawaii HCCN

Founded in 1989, the Hawai‘i Primary Care Association has been working on behalf of vulnerable populations throughout our state, and helping to establish access to primary health care services for everyone. HPCA has developed strong, productive partnerships with providers, health care organizations, lawmakers, policy officials, and public advocates to improve quality and access for nearly 150,000 people. As a part of the Hawaii PCA, the Hawaii HCCN supports 10 network members.
