Colorectal Cancer Screening Change Package

Released in July of 2016, this change package is comprised of tools and resources pertinent to colorectal cancer screening from authoritative resources on the topic and organized by the change package outline. Some of these approaches and tools were used in the NACHC Improving Data Integrity, Access, and Analysis Project, which focused on applying data-driven strategies to improve colorectal cancer screening. The change package structure and organization aligns with TMIT Consulting’s CDS/QI Worksheets (available for download here), in that they are both organized around Key Foundations, Population Health Management, and Individual Patient Supports. Change packages like this one provide vetted enhancement strategies and tools that support the analysis by helping teams understand ‘What do successful approaches to the target look like?’
