Telehealth and Chronic Care Management

Telehealth and Chronic Care Management

FQHC Webinar Series

Learn how to get started by reviewing what you need to consider when developing your telehealth program, from selecting equipment to policy issues you need to consider. Hear from a FQHC success story on how they did it.

With the increasing number of reimbursement codes and interest from the Center for Medicare and Medicaid Services (CMS) in paying for Chronic Care Management (CCM) services in Medicare, opportunities are available to CHCs to take advantage of utilizing remote patient technology (RPM) to address their community’s needs. By managing chronic conditions, more serious health episodes may be avoided, leading to potential cost saving and better quality of life for patients. Hear about these opportunities and how to go about taking advantage of them.

Speakers:

  • Kathy Wibberly, Director, Mid-Atlantic Telehealth Resource Center
  • Howard Chapman, Director of Programs and Development, Tri-Area Community Health
