BFHC implemented the i2i Tracks Population Health Management Platform, which connects to EHRs to extract and aggregate clinical and financial data, identify at-risk populations, assign personalized actionable care pathways, and engage patients in team-based care.  BFHC’s goal was to improve the health of their patient population.  As a result of their population health implementation project, BFHC earned the earned the prestigious Level 3 NCQA-PCQH accreditation within just 24 months.
