The Alabama Primary Health Care Association (APHCA) was established in 1985 as a non-profit, professional trade association whose mission is to strengthen and expand Alabama’s health center network through leadership, services, and collaboration so that Alabamians have access to improved outcomes through coordinated health care. As part of APHCA, there are 16 CHC members providing care across Alabama.

Founded in 1972, Golden Valley Health Centers is dedicated to improving the health of our patients by providing quality, primary health care services to people in the communities we serve regardless of language, financial or cultural barriers. As a private non-profit Federally Qualified Health Center system they operate clinical and dental sites in Merced and Stanislaus counties.

Incorporated in 1998, Central Valley Health Network is comprised of 15 Community Health Center corporations designated as Federally Qualified Health Centers (FQHCs). CVHN was established to conduct advocacy at national, state, and local levels on behalf of its Central Valley membership of FQHC’s and their unique patient population specific to the Valley. CVHN’s network is also recognized nationally as one of the largest network of Migrant Health Centers serving rural, farm-working populations and their families.

Maine Primary Care Association (MPCA) is a membership organization representing the collective voices of Maine’s Community Health Centers - referred to as Maine’s health care safety net. Founded in 1981, MPCA has provided technical assistance and training, housed relevant programs and services and advocated on behalf of Maine’s safety net and the thousands of patients it serves.

Colorado Community Managed Care Network (CCMCN) is comprised of 19 Community Health Centers. CCMCN was founded in 1994 to respond pro-actively to the advent of mandatory Medicaid managed care, and has evolved to a multi-faceted organization focusing on managed and accountable care, health information technology, and clinical quality improvement programming.

Since 1996, Community Care Network of Virginia, Inc. (CCNV), has been a trusted support system, training partner and business process expert for health providers, including Federally Qualified Health Centers (FQHC), private practices, hospitals, managed care organizations, and provider networks. Providing integrated, network-based services and programs to healthcare clients to help them run their businesses better so that they can focus on patient care. This includes the centralized practice management system, help desk, and support infrastructure that we established in 1999.

The Community Clinic Association of Los Angeles (CCALAC) was founded in 1994, alongside their 62 memebers  they share a common mission to promote community clinics and health centers as providers and advocates for expanding access to quality comprehensive health care for medically underserved people in Los Angeles County.

The Community Clinic Consurtium was established in Contra Costa County in 2004, and provides representation and support to 26 non-profit community health center sites and their patients. The Consortium works at the county, state and federal government levels to advocate for funding and policy changes that help health centers meet the diverse health care and social service needs of almost 200,000 patients annually. The Consortium also serves as a local resource for clinic staff, clients and the community.

The Community Health Access Network (CHAN) is the only Health Center Controlled Network (HCCN) in New Hampshire.  Established in 1995, CHAN has developed and supports an integrated clinical and administrative system infrastructure for its ten Federally Qualified Health Center (FQHC) members which includes 3 Healthcare for the Homeless Programs.

Founded in 2009, Community Health Best Practices, LLC is a nonprofit network comprised of fifteen of the nation’s leading community health centers.  Our Members operate federally qualified health centers (FQHCs) in Oregon, Washington, Arizona, California, Colorado, New Mexico, Texas, Hawaii, Connecticut, New York, Massachusetts, and Puerto Rico, and they provide comprehensive primary care and behavioral health services to all patients regardless of the patients’ ability to pay for such services.

Incorporated in 1989, the Community Health Center Association of Connecticut (CHCACT) is a not-for-profit service organization dedicated to strengthening and supporting the clinical and administrative operations of its 16 member community health centers across Connecticut.

Founded in 1996, Community Health Center Network (CHCN) is committed to excellent, affordable care for underserved communities of the East Bay. We are a non-profit Medi-Cal managed care organization, providing business administrative support for community health centers to focus on what matters most–patients. CHCN was formed to participate more effectively in newly-launched state managed care programs. The formation of CHCN built upon 20 years of collaboration in health policy and advocacy work through the Alameda Health Consortium.

Founded in 1999 by visionary health care leaders, CHCA provides 17 Federally qualified Health Centers (FQHCs) and other healthcare organizations with solutions to advance health information technology adoption including implementation and project management, infrastructure hosting, application training, report development, on-going application support services, and consulting and education for various related initiatives.

CHIP, Inc. was founded in 1997 as the only statewide network of community health centers in the State of Maryland. As a leader and advocate for quality community health care in Maryland, CHIP provides community health centers with the business expertise to achieve the shared goal of quality improvement in the care they deliver.  In doing so, we give these centers access to higher levels of sophisticated health information technology (HIT) and management services than they could achieve and maintain individually.

The Community Health Care Association of New York State's (CHCANYS) purpose is to ensure that all New Yorkers, including those who are medically underserved, have continuous access to high quality community-based health care services including a primary care home. To do this, CHCANYS serves as the voice of community health centers as leading providers of primary health care in New York State.

Founded in 2013 The Federally Qualified Health Center Urban Health Network (FUHN) is an organization of ten Twin Cities FQHCs working in partnership with Minnesota’s Department of Human Services on Medicaid health care reform efforts.  These efforts include FUHN’s participation in a Integrated Health Partnership Accountable Care Organization demonstration project. Through this demonstration project FUHN participant FQHCs hope to further enhance the health care provided to their Medical Assistance patients by coupling our primary care efforts with expanded use of technology.

The Georgia Association for Primary Health Care, Inc. (GAPHC) is a membership organization for all Federally Qualified Health Centers (FQHC) in the state of Georgia. GAPHC was founded in 1977 and is a statewide organization located in the Atlanta Metropolitan Area. GAPHC is quite familiar and knowledgeable about the relevant rules and regulations governing FQHC – Section 330 organizations. GAPHC is very active in the development of various operational models for FQHC organizations and provides direction and leadership for required community development activities that are related to both the development of new start FQHC organizations and operations of existing FQHC organizations.

Founded in 1989, the Hawai‘i Primary Care Association has been working on behalf of vulnerable populations throughout our state, and helping to establish access to primary health care services for everyone. HPCA has developed strong, productive partnerships with providers, health care organizations, lawmakers, policy officials, and public advocates to improve quality and access for nearly 150,000 people. As a part of the Hawaii PCA, the Hawaii HCCN supports 10 network members.

The Kansas Association for the Medically Underserved (KAMU) exists to achieve equitable access to high quality health care for all Kansans. As the Primary Care Association (PCA) for the state of Kansas, KAMU has been supporting and strengthening its member organizations through advocacy, education, and communication since 1989. KAMU represents a diverse group of public and private nonprofit primary care clinics, Federally Qualified Health Centers (FQHCs), local health departments, and rural health clinics, as well as private individuals who have a keen interest in health care for Kansas’ underserved populations. Together, the member clinics are known as the Kansas Safety Net, and they provide medical, dental, and behavioral health care regardless of the ability to pay.

Founded in 2007 by six health centers and the Community Health Care Association of New York State (CHCANYS), HealthEfficient now serves 18 health centers across the eastern U.S. Our data reporting and benchmarking solutions contain demographic and clinical information on nearly 260,000 patients served network-wide. Since 2008, CHCANYS has offered super-user and end-user training to health centers who use eClinicalWorks, helping staff master the skills they need to get more work done, faster—and focus more on patient care. If new to eClinicalWorks, they can help health centers train their staff on the system right from implementation.

Formed in 1985, The Association for Utah Community Health (AUCH) has represented Utah's Health Centers and their patients. As the Primary Care Association in the state, we provide over 12,000 hours of training and technical assistance to 13 Health Centers and 5 affiliate members each year.

Founded in 1994, Health Choice Network, Inc. is a successful nation-wide collaboration among health centers, health center controlled networks and partners. By providing key business services, strategic initiatives and the latest in health information technology, participants can improve patient outcomes through increased efficiencies and more accessible care. With 27 health centers in nine states serving approximately one million patients, Health Choice Network, Inc. is recognized as a leader in the integration of health information technology among health centers and safety net providers.

Founded in 1983 as a membership organization of Federally Qualified Community Health Centers, the Health Federation of Philadelphia continue to partner with members to improve the quality and accessibility of primary health care, including behavioral support, for vulnerable populations in Southeastern Pennsylvania. They support the functioning of members with collective advocacy, professional development, individualized consultation, and shared learning.

The Arizona Alliance for Community Health Centers (AACHC) is the Primary Care Association (PCA) for the State of Arizona since 1985. All states have one designated PCA to advance the expansion of the Health Center Program and advocate for the health care interests of the medically underserved and uninsured. AACHC, as a result, has many programs to assist member community health centers and the disadvantaged populations they serve. These programs vary from centralizing financial information and educational opportunities for members to peer networking committees for sharing of best practices.

Established in 2011, The Health Center Association of Nebraska (HCAN) is a nonprofit primary care organization whose mission is to enhance and expand access to quality, community-responsive healthcare for Nebraska’s medically underserved and uninsured. Members of HCAN are federally-qualified Community Health Centers (CHCs) that provide medical, dental, and mental health services for over 63,000 Nebraskans.
