Having rapid access to data in health centers is critical to managing clinics, using operational data to improve care, and reporting quality measures. Quality, accessible data in health care can do more than feature in reports and accountability systems: effective communication of information can improve quality of care, alignment to evidence-based care principles, and data-informed decision making. Charts, graphs, maps, and other data visualizations play a key role in making information accessible. Visualizations make the patterns in large datasets rapidly apparent, showing when numbers are going up or down, performance compared to goals, comparison between patient groups, and more. The purpose of this resource is to provide a comprehensive guide to user-centered dashboard design for health center staff.

Value-based care focuses on improving patient outcomes while optimizing total cost of care. For health centers seeking to improve their internal processes for value-based care, effective data visualization can play a significant role in understanding, monitoring, and promoting value-based care practices. This resource details how data visualization can be employed to support value-based care and includes suggestions on how to depict measures and trends using data dashboards.

This spreadsheet developed by the HITEQ Center provides a crosswalk of Clinical Quality Measures and their electronic specifications as defined in the 2023 update for Eligible Professionals (Clinicians). Fields include the crosswalk of measures with related information about CMS, NQF, and MIPS ID, and Telehealth Eligibility, as well as inclusion in HRSA BPHC Uniform Data System (UDS) CY2023, Million Hearts, CMS Quality Payment Program (QPP) - Performance Pathway (APP) Measures, Medicare Shared Savings Program (MSSP)/ CMS ACO Shared Savings Program, CMS Core Set (Child Core Set Medicaid / CHIP): HEDIS Specified, CMS Core Set (Adult Core Set Medicaid): HEDIS Specified, Core Quality Measures Collaborative (ACO / Primary Care). The HITEQ Center planned a series of discussions about what is on the horizon-- this includes the people, processes, and technologies related to UDS+, environmental impacts and environmental determinants of health (EDoH), and Artificial Intelligence (AI). Each session will be interactive and engaging and include time for health center sharing. 

This series was open to community health centers throughout the nation. If you are from a health center and are interested in enhancing your understanding of UDS+ demographic and clinical quality measures, environmental impacts and EDoH, and/or AI, you should watch this series! This might include EHR or IT staff, clinical or operations staff, leadership, quality and population health staff, and/or clinical staff. Registration for each session is below.
