Lessons Learned: Implementing and Expanding Social Needs Screening Programs in Health Centers - Session 5: Learning Lab

HITEQ Learning Collaborative Series

Is your health center currently in the process of considering, implementing, or revamping a social needs screening program? Join this learning collaborative by the HITEQ Center to learn about health center promising practices and key considerations to support the successful collection, monitoring, and addressing of social needs data. During the series participants will explore the levels of maturity in the social needs screening implementation process. Participants will gain information on concrete strategies and IT solutions that will help to improve internal systems, such as EHR utilization and care team workflows, and increase their capacity to advance individual and population-level health.  Health center exemplars will be showcased.

The levels of maturity include: 

  • Level 1: Coming to Consensus

  • Level 2: Implementing a Social Needs Screening Tool

  • Level 3: Responding to Positive Screens

  • Level 4: Monitoring and Using Data

Who should attend/join?

If you work at a health center and contribute to social needs screening configuration, documentation, or evaluation, you should join this series! This might include EHR or IT staff, clinical or operations staff, leadership, quality and population health staff, and/or clinical staff. 

What should participants expect?

  • There is no fee to participate.

  • All sessions will be hosted by HITEQ using Zoom Meeting. Each will start at 1:00 pm ET/ 10:00 am PT and will be 60 minutes.

  • Participants will: join four core learning sessions and one optional learning lab, participate actively throughout, and complete evaluations to provide feedback on each session.

  • Sessions will be interactive and engaging. Participants are encouraged to join by video.

  • Participants will be asked to join a brief follow-up session 3 months after the series. 

  • The learning collaborative is open to health center staff nationwide. PCAs and HCCNs are welcome to attend with a health center in their network.

  • Applying or registering for the series indicates that participants have obtained health center leadership approval and support to participate in the series.

What will you get from this Learning Collaborative?

After participating in this series, attendees will be able to:

  • Assess the primary considerations at each level of social needs screening program maturity and how they might relate to each health center’s unique environment

  • Anticipate implementation challenges and identify potential IT and workflow solutions for advancing social needs screening programs

  • Identify strategies for leveraging health IT to successfully collect social needs screening data

  • Identify strategies for leveraging health IT to successfully monitor and address positive social needs screens

Session 5: Learning Lab: Health Center Sharing on Actions and Progress in Implementing and Advancing Their Social Needs Screening Program

During this session, participants will share their takeaways from the learning collaborative, and plans and action steps to advance their social needs screening program.
