Session 2: Hearing from CHWs on Challenges and Successes with Joining the Team

Join the HITEQ Center for a two-part training on integrating community health workers (CHWs) as strategic team members in the age of value based care. In the first session, participants will explore the role of CHWs in data-driven activities essential to value based care, including outreach and proactive care management, and how these rely on successful use of the health center's EHR and other systems. The second session will feature a conversation with CHWs, sharing their challenges and successes in becoming team members in the current landscape of value based care. Please note that registration for the two sessions is separate.

Learning Objectives
After participating in this session, attendees will:

  • Identify the key roles of CHWs in data-driven activities essential to value based care.
  • Describe effective strategies for integrating CHWs into healthcare teams, focusing on strategies that enhance collaboration and improve patient outcomes in value based care settings.
  • Gain firsthand insights from CHWs regarding their experiences, challenges, and successes in integrating into value based care teams.
