Modified Stage 2 Meaningful Use Objective 5 for Eligible Providers

Starting in 2016, providers will be reporting using either the modified Stage 2 or Stage 3 of CMS Meaningful Use (MU) requirements. Of the ten MU measures for Eligible Providers (EP), several related to electronic information exchange across systems. However, CMS MU Objective #5 specifically addresses Health Information Exchange (HIE) requirements for EP. HIE under this measure can be achieved by conducting patient transitions of care from (1) one setting of care to another care setting or to another provider, or (2) via referrals from the EP to another provider.
